A Telemanagement program called “well@home” is available for patients receiving services from Valley Home Care. A private pay option is also available for patients no longer on service or for patients who have never received Valley Home Care services.

Patients receive a lightweight portable monitor equipped with built-in devices that record blood pressure, heartbeat, blood oxygen levels and weight. With a color touch screen, easy to read graphics and clear voice instructions, the monitor is patient friendly.

In addition, the well@home monitor can be customized to ask and report patients’ specific questions about their condition and current symptoms. Reminders for patients to take medications on schedule may be added to the daily tasks on the well@home screen. Patient specific education is also programmed to each individual monitor.

The well@home monitor is connected to the patient’s phone line and automatically transmits all information entered by the patient daily via an 800 telephone number. The information is sent to a secure web-site that is reviewed daily by a telemonitor nurse. Communication is maintained with physicians to keep them apprised of changes in their patient’s condition. Doctors can also view their patient’s records by logging onto the secure well@home web-site.
